Failure, Malfunction & Defect Reporting

The Design Organisation under the approval Number EASA.21J.316 has established procedures to fulfil the requirements of EASA Part 21, 21A.3. This requires any failure, malfunction or defect, which is or might be in direct conjunction with a change to the product designed by Gate V to be reported from the operator, the aircraft owner or any other person or organisation having such information available.We therefore ask to fill in the following form and submit it to us.
